Mr. Spencer,

Pursuant to the Indiana Access to Public Records Act, Ind. Code § 5-14-3, I am requesting electronic copies of records maintained by the City of North Vernon and North Vernon Municipal Utilities concerning fire hydrants that are currently not fully operational or reliably available for firefighting use.

Please provide any existing lists, spreadsheets, database records, maps, reports, inspection records, testing logs, maintenance logs, work orders, repair tickets, internal notices, or other records sufficient to identify each hydrant currently recorded as any of the following:

Out of service;
Inoperable or partially inoperable;
Broken, damaged, leaking, shut off, bagged, tagged, or disabled;
Awaiting inspection, repair, or replacement; or
Otherwise not reliably available for normal firefighting use.
For each affected hydrant, please provide records showing, where maintained:

The location of the hydrant, including the street address, nearest intersection, or other identifying location;
Any hydrant identification number or map reference;
The date the hydrant was first reported, observed, tested, or recorded as not fully operational;
The nature of the problem;
The current status of the hydrant;
The date any work order or repair request was opened;
Any scheduled, estimated, or completed repair date; and
Any records showing whether the North Vernon Fire Department or any other emergency-response agency was notified of the hydrant’s condition.
This request seeks existing records only. It does not ask the city or utility department to create a new document, compile a new report, or answer questions that are not already answered by records maintained in the ordinary course of business.

If the responsive information is stored in a database or spreadsheet, please provide an electronic export of the responsive records. If responsive records are maintained in more than one format or location, please provide the existing responsive records from each source.

If no responsive records exist, please state that in writing. If any responsive record or portion of a record is withheld or redacted, please identify the specific statutory basis for the withholding or redaction.

Please acknowledge receipt of this request and provide an estimated date for production. Electronic delivery by email is preferred.
